Middelharnis has about 17,554 residents and lies on the island of Goeree-Overflakkee in South Holland, within the gemeente Goeree-Overflakkee since 2013. The town functions as the island's administrative and commercial centre. The historic harbour, once active in fishing and trade, opens onto the Haringvliet. The Raadhuis and surrounding streets on the Ring form a well-preserved 17th and 18th-century core.
The island was historically isolated, connected to the mainland only by ferry until the Haringvlietdam and connecting roads were built as part of the Delta Works. Agriculture, particularly potatoes and onions, dominates the flat polder landscape. The Grevelingen lake to the south is popular for sailing and diving. The Reformed Protestant community remains influential in the island's social fabric.
Rotterdam is about 40 kilometres northeast. Hellevoetsluis lies roughly 20 kilometres north across the Haringvliet. Ouddorp on the western coast is approximately 15 kilometres west.
